Caregiving doesn’t usually begin with a decision.
It begins with a moment.
A phone call from the hospital. A fall that changes everything. A discharge date that arrives faster than expected. Suddenly, families across Nova Scotia and Prince Edward Island find themselves responsible for far more than they were prepared for.
Many family caregivers tell us:
“I thought someone would explain what to do next.”“I didn’t realize how much would fall on me.”“I’m exhausted — and we’re only at the beginning.”
When caring starts to feel overwhelming, it’s not because families are failing. It’s because they’ve been handed responsibility without a roadmap.
That’s why Remember When HealthCare (RWHC) created a workshop series — to support families through education, stress relief, and personalized care solutions, not to sell services.
A Nurse-Led Workshop Series Designed to Guide Families — Not Overwhelm Them
This 4-Part Aging in Place workshop series was intentionally designed as four standalone sessions that work together to support families through different stages of caregiving.
You don’t need to attend all four.
Families are encouraged to attend the sessions that match where they are right now — whether they’re planning ahead or responding to a sudden change.
Together, these workshops help families move from uncertainty to confidence.
Workshop 1: Understanding Care Options & Funding in Nova Scotia
“Making sense of what’s available — and what’s not.”
This workshop helps families understand how care is structured and funded in Nova Scotia and PEI.
Topics include:
Public, private, and community-based care options
How Continuing Care works — and where gaps exist
Home care, respite, assisted living, and long-term care
Assessments, referrals, and waitlists
Funding options, tax credits, and caregiver benefits
What families can do while waiting for services
Families often leave this session feeling relief — because the system finally feels understandable.
Workshop 2: Navigating Transitions from Hospital to Home
“What to expect, what to ask, and how to prepare.”
Transitions from hospital to home are where many families feel the most pressure.
This workshop supports families through:
What discharge planning actually includes
Questions to ask before leaving hospital
Temporary versus long-term care needs
When short-term home health care services can help
Adapting the home quickly after discharge
Creating a realistic care plan
Reducing the risk of readmission
Families gain confidence by knowing what’s normal — and what help exists.
Workshop 3: Preparing the Home for Aging in Place
“Practical ways to make your home safe, comfortable, and livable.”
This workshop focuses on creating environments that support safety and independence.
Participants learn about:
Fall prevention and home safety
Bathroom, kitchen, and bedroom adaptations
Assistive devices and funding options
Seasonal risks specific to NS & PEI
Technology that supports independence
Planning ahead helps families avoid crisis-driven decisions.
Workshop 4: Building a Support System & Having ‘The Talk’
“Care takes a village — let’s talk about how to build yours.”
Caregiver burnout often comes from trying to do everything alone.
This workshop helps families:
Build a realistic support network
Identify their personal “care circle”
Navigate difficult family conversations
Access community and respite resources
Plan ahead for medical and legal decisions
Families leave understanding that support doesn’t mean giving up control.
