Jocelyn is one of the Community Care Managers at Remember When HealthCare, supporting clients and families across Prince Edward Island, including Charlottetown, Summerside, Stratford, Cornwall, Kensington, Montague, Souris, and surrounding communities. She completes new client assessments, develops and updates personalized care plans, and works closely with clients, families, caregivers, and healthcare providers to ensure every individual receives compassionate, coordinated care that supports safe and independent living at home.
My name is Jocelyn, and I am a Registered Nurse (RN) living in Mermaid, Prince Edward Island. Originally from Morell, PEI, I have been passionate about nursing for more than 30 years and am dedicated to helping individuals and families access the care and support they need to remain healthy and independent.
I graduated from the PEI School of Nursing in 1992 and later completed my Bachelor of Nursing through Athabasca University. Throughout my career, I have worked in Medical-Surgical Acute Care, Mental Health and Addictions, Community Home Care, Palliative Care, Long-Term Care, Infection Prevention and Control, and with Veterans Affairs. I also hold certifications in Hospice Palliative Care, Gerontology, and Advanced and Diabetic Nursing Foot Care.
As Community Care Manager for Prince Edward Island, I work closely with clients, families, caregivers, and healthcare professionals to develop personalized care plans and ensure every client receives compassionate, coordinated support. I believe in building trusting relationships, listening to what matters most to each person, and helping families navigate their homecare journey with confidence.
I chose to join Remember When HealthCare because of its nurse-led, person-centered approach and commitment to helping people age in place safely and comfortably. It is rewarding to be part of a team that shares my passion for delivering high-quality care with kindness, professionalism, and respect.
Outside of work, I enjoy cooking, baking, gardening, painting, spending time outdoors, and walking the beaches of PEI in search of sea glass. I look forward to supporting clients and families across Prince Edward Island as they continue living safely and independently at home.
