Having the right support can make everyday activities easier while allowing you to maintain choice and control over your life.

Remember When HealthCare is approved by Nova Scotia’s Disability Support Program to provide supports and services to participants receiving Individualized Funding.

We provide services throughout Nova Scotia, including rural communities. Our nurse-led team takes the time to understand your routines, communication preferences and personal goals, with an emphasis on familiar faces, consistent relationships and person-directed support.

What Is Individualized Funding?

Individualized Funding, also called IF, is a service model offered through Nova Scotia’s Disability Support Program.

It provides eligible participants with funding to purchase approved supports and services that help them live at home, participate in their communities and work toward their personal goals.

Individualized Funding gives participants greater choice, flexibility and control over the supports they receive and who provides them.

The Disability Support Program determines eligibility, completes the required assessment and approves each participant’s funding and DSP Support Plan.

Who This Service Is For

Our services are available to participants who receive Individualized Funding through Nova Scotia’s Disability Support Program.

This service may be suitable for:

Adults living with disabilities who need assistance with everyday activities

Participants who want support at home or in their community

People developing or maintaining daily living skills

Individuals who want more structure and consistency in their routines

People preparing to live more independently

Participants working toward social, recreational, educational, volunteer or employment goals

Families or authorized representatives helping a participant arrange approved services

Your DSP representative can explain your approved funding and the supports and services included in your DSP Support Plan.

How Individualized Funding Works

The Disability Support Program determines your eligibility and funding through its assessment and support-planning process. If you are approved for Individualized Funding, you will receive funding to purchase the eligible supports and services included in your DSP Support Plan.

You or your authorized representative receive and manage the approved funding. You can then choose Remember When HealthCare to provide eligible services and pay RWHC for the services you receive.

Remember When HealthCare provides your approved supports and services but does not hold, manage or administer your Individualized Funding.

What Is Included

Your services will depend on your approved DSP Support Plan, available funding and individual goals.

Personal care and other activities of daily living

Meal planning and meal preparation

Grocery shopping and errands

Laundry, light housekeeping and household routines

Medication reminders and support with agreed medication routines

Transportation and community access

Establishing and maintaining daily routines

Developing and strengthening everyday living skills

Social and recreational activities

Maintaining personal and community connections

Support with education, volunteering or employment-related activities

Respite and family wellness supports included in the approved plan

Other eligible disability-related supports approved through DSP

Nursing or other medical support may only be purchased using Individualized Funding when specific funding for that medical support is included in your approved DSP Support Plan.

Meaningful support is not limited to completing tasks. It can also help you build confidence, strengthen your skills and participate more fully in your community.

Your support might include preparing a meal together, organizing your weekly routine, attending an appointment, practising how to use transportation or participating in a community activity.

We work alongside you by providing encouragement and practical assistance while creating opportunities to use and strengthen your skills.

Who Provides This Support

Your services are delivered by RWHC team members who are matched with your needs, preferences and approved supports, with guidance from our nursing team. We take time to understand your communication style, routines, personal goals and how you prefer your support to be provided.

Consistency matters. Familiar faces and predictable routines can help build trust and create comfortable, reliable relationships.

Family members, authorized representatives, friends and other trusted people can be involved when you choose.
