Supported independent living helps adults with disabilities manage everyday activities while maintaining choice and independence. Support can build on existing skills, establish comfortable routines and make it easier to participate in community life.
Remember When HealthCare is an approved Independent Living Support (ILS) service provider serving all of Nova Scotia, including rural communities. Our nurse-led team works alongside you, with an emphasis on familiar faces, consistent relationships and your personal goals.
What Is ILS?
Independent Living Support is offered through Nova Scotia’s Disability Support Program (DSP). It provides funding for approved support hours to help eligible people with disabilities live independently or semi-independently in their own homes and communities.
Support is person-directed, meaning your goals, preferences and choices guide your plan. This may involve learning a new skill, maintaining a familiar routine or receiving practical help with activities that are difficult to manage alone.
The Disability Support Program determines eligibility and funding through its assessment process.

How ILS Funding Works
The Independent Living Support program provides funding for approved support hours based on each participant’s assessed needs and circumstances. The Disability Support Program determines eligibility, funding and the level of support approved.
Your DSP Care Coordinator can explain what your funding covers and discuss a review if your needs change. Remember When HealthCare develops your support plan with you within those approvals.
What Is Included
Support focuses on practical daily activities, personal goals and community participation.
Depending on your approved support plan, ILS services may include:
Meal planning, grocery shopping and meal preparation
Laundry, light housekeeping and household organization
Developing and maintaining daily routines
Medication reminders and support with agreed medication routines
Community participation and transportation support
Social activities, hobbies and maintaining personal connections
Support with education and employment-related routines
Connecting with professional and community supports
Remember When HealthCare does not provide financial management services through ILS, including holding or administering participants’ money or paying bills on their behalf.
Support at Home and in the Community

Independent living involves both managing everyday tasks at home and participating in life outside it.
Your support might include preparing a meal together, organizing a weekly routine, practising how to get to an appointment or attending a community activity.
We work alongside you, providing encouragement and practical assistance while creating opportunities to use and strengthen your skills. The activities and schedule reflect your approved support plan.
Who Provides This Support
ILS support is delivered by team members matched to your needs and preferences, with guidance from our nursing team. We take time to understand your communication style, routines and how you prefer assistance to be provided.
Consistency matters. Familiar faces help build trust and make it easier to work towards goals together. Family members, friends or other trusted people can be involved when you choose.
When Independent Living Support Is Helpful
ILS can be helpful when you are preparing to live on your own, finding certain household tasks difficult to manage or looking for more structure in your day.
It can also support goals such as becoming more confident preparing meals, managing appointments or participating in activities outside the home. If your circumstances change, your DSP Care Coordinator can review whether your approved supports continue to meet your needs.
