About the event
This free, in-person workshop series is designed for seniors, families, family caregivers, and anyone planning ahead for aging in place. Hosted at the Charlottetown Library, each nurse-led session focuses on education, awareness, and practical guidance around care planning, navigating the system, and preparing for the future. Workshops are facilitated by nurses Sarah Fox, BScN RN, and Kelly St. Denis, LPN. Participants are welcome to attend all four workshops or choose only the sessions most relevant to their needs—each workshop stands on its own and includes a free takeaway resource.
Workshop Overview
April 21: Workshop 1: Understanding Care Options & Funding
Making sense of what’s available — and what’s not.
This workshop helps seniors, families, and caregivers better understand the care landscape in PEI. Topics include public, private, and community-based care options, how government funding works, common system gaps, and available grants, tax credits, and funding supports. A free worksheet is included, along with access to our comprehensive guide outlining grants and funding options for aging in place.
April 28: Workshop 2: Navigating Transitions from Hospital to Home
What to expect, what to ask, and how to prepare.
This session focuses on the transition from hospital to home and what families should know before discharge. Participants will learn how to plan for temporary and long-term care needs, understand discharge planning, and prepare the home for recovery. A free worksheet is included, along with access to our guide covering grants, funding, and available supports.
May 5: Workshop 3: Preparing the Home for Aging in Place
Practical ways to make your home safe, comfortable, and long-term livable.
This workshop explores practical strategies for adapting the home to support aging in place. Topics include fall prevention, home safety, assistive devices, environmental considerations, and when it may be time to reassess living arrangements. Participants receive a free worksheet and access to our funding and grants guide.
May 12: Workshop 4: Building a Support System & Having ‘The Talk’
Care takes a village — let’s talk about how to build yours.
This session guides families through the emotional and practical aspects of planning for aging. Topics include building a care circle, identifying supports, and approaching conversations about independence, safety, and future wishes. A free worksheet is included, along with access to our comprehensive guide to grants and funding options.
This series provides clear, practical information to help individuals and families make informed decisions about aging in place. Registration is free. Please note you will need to register for all 4 sessions individually if you wish to attend more than one.
